Please arrive 15 minutes ahead of your first scheduled appointment, so that we may complete your patient registration information.
For your convenience, you may download and print copies of most of our new patient registration forms.
Please complete these prior to your first appointment, and bring them along with your current insurance card.

You should plan for your first appointment to last approximately one hour to allow sufficient time for a thorough evaluation of your condition.
If possible, dress in comfortable clothing such as a t-shirt, shorts or sweatpants. Gowns are shorts are available for use if necessary.

Avante Physical Therapy accepts coverage from many of the major insurance providers.
Please check our list of accepted insurances to confirm that your plan is included.
If you do not see your plan listed, we can provide you with an out-of-network referral.

We are happy to provide insurance filing for you.
Payment of your co-pay is expected for each office visit, and we accept cash, personal check, Visa and MasterCard.

Not only do we provide personal, one-on-one care, in plain, easy to understand English, we can also communicate in Dutch, Afrikaans, French and Spanish. Please indicate your special needs when you call for your first appointment.
